Vaccination Policy
Our practice strongly believes in administering childhood vaccination for disease prevention. In support of our beliefs and for the health and well-being of our patient population, we are not accepting new patients to the practice who do not comply with the minimum recommended vaccines: DTap, HiB and Prevnar 13 by the age of 1 and MMR by the age of 2.
In Case of Emergency
If your child is experiencing a life-threatening emergency, CALL 911 for immediate help. Urgent problems during regular office hours can be handled by contacting the office. Notify the staff member who answers the phone that you have an urgent problem and your call will be handled immediately, or come in to the office right away. The physicians can be reached for urgent problems after hours by calling our answering service, at (702) 363-3000 and the service will page the physician on call. The provider will then return your call at the number you provide.
Important
Do not call from a phone that does not accept blocked calls, as physicians will be calling from a phone with a blocked number. Also, please do not call for routine calls after hours (such as a dose of medication): please respect our physicians’ time with their families. These calls are subject to charge. For any urgent care issues that occur after midnight, patients are encouraged to be seen by a health professional.
Download Forms
For your convenience prior to visiting our office, print all the necessary forms on our Office Forms and Policies page. Complete the information and bring it with you to your first visit.
Did You Know...
Yearly Exam is Recommended
All children should be examined at least once yearly throughout their lives. Although children are usually very healthy, an ounce of preventative medicine (including vaccinations) can help keep your child healthy for years to come.
If Someone Else is Bringing Your Child
For the safety of our children, we require a note of authorization from you if someone else will be bringing your child in to be seen. Please ask a staff member how a notarized form can eliminate the need for these notes for an entire year.
Physicians On-Call
Our on-call physicians can be reached throughout the day though by calling 363-3000. Please reserve after-hours calls for URGENT and EMERGENCY calls only.
Medication Refills
Calls regarding dosage or refill of medications should be made during regular office hours or to our non-emergency nurse line at 214-0900.
Fever in Newborn Babies
The following symptoms are considered emergent in newborn babies that are less than two months old:
- Fever greater than 100.4 F (Take baby’s temperature rectally if baby feels warm and temperature is normal under the arm).
